This game does not come with a cover or a manual. It includes an original case, disc, disc tray, and back cover. The case has a single crack on the front as well as a chip with missing plastic on the side. The case is in poor condition but everything else is in good condition.

All conditions marked as 'Disc Only' will include the disc only and a blank DVD case.
